Hydration and fibre for constipation during GLP-1 treatment

GLP-1-type treatments, such as semaglutide and tirzepatide, can slow digestive transit in some people. Constipation may therefore occur particularly at the start or after a dose increase.

Why does it happen?

Reduced appetite can lower the amount of food, fibre and fluids consumed. Medicines can also alter digestive timing. If you eat much less, drink little water and move less, constipation becomes more likely.

What can you try if your doctor allows it?

  • gradually increasing fibre: vegetables, pulses, oats and seeds;
  • regular hydration throughout the day;
  • walking after meals and gentle physical activity;
  • small meals that still include protein and fibre;
  • medical advice before laxatives, especially if you have digestive conditions.

A Mediterranean-style diet is useful because it provides fibre and unsaturated fats, but portions need to be adjusted to tolerability.

Warning signs

Contact your doctor if you develop intense abdominal pain, persistent vomiting, severe bloating, blood in the stool, inability to pass gas or constipation that worsens rapidly. Do not increase the dose or combine treatments without advice.
